Outlook stores all these settings using
a single configuration file, called OUTCMD.DAT. OUTCMD.DAT also stores the
custom changes made on the Outlook default toolbars and menus.
OUTCMD.DAT file can become corrupt as a result of hard disk problems, Outlook malfunctioning or corruption caused by 3rd party add-ins. When OUTCMD.DAT is corrupted, Outlook can show one of the above symptoms numbered 1 through 3.

Restoring Outlook
Notice: this procedure will reset Outlook to its DEFAULT views and
it will NOT restore CUSTOM toolbars or menus.
To restore Outlook views, toolbars, menus or settings, you need to reset the content of the OUTCMD.DAT file. To do this, follow the steps below:
1. Close Outlook
2. Browse to the OUTCMD.DAT file in the users profile
Windows XP: Outlooks default path is "C:\Documents and Settings\USER\Application Data\Microsoft\Outlook"
Windows 7: Outlooks default path is
"C:\Users\user\AppData\Roaming\Microsoft\Outlook\outcmd.dat"To restore Outlook views, toolbars, menus or settings, you need to reset the content of the OUTCMD.DAT file. To do this, follow the steps below:
1. Close Outlook
2. Browse to the OUTCMD.DAT file in the users profile
Windows XP: Outlooks default path is "C:\Documents and Settings\USER\Application Data\Microsoft\Outlook"
3. Rename the OUTCMD.DAT
4. Restart Outlook.
Launching Outlook will restore the default OUTCMD.DAT file and you will be able to use Outlook with its default view settings once more.
